Family Supports

Thrive Upstate provides Case Management and other Family Supports to SC Department of Disabilities and Special Needs eligible individuals living in Greenville County helping to coordinate care through all of life’s stages.

Navigating the service delivery system from early childhood, through the school years, and into adulthood can be very challenging for people with lifelong disabilities and their families.  Our Family Supports department has staff who are trained to assess needs and link people to community resources connecting them to work opportunities, in-home supports, financial assistance and any other special services that are needed.
